What To Include in an IT Support Ticket

  • Requestor: Your Company Email. To Launch a ticket for someone else, place their email here. In case they do not have an email address, all correspondence will go to this email address.
  • Subject: What this Ticket is about. It could be a question, or Issue you are needing help with, or an Incident problem you are experiencing. If this is urgent, please include the keyword "Urgent" in your subject line or in the description.
  • Category: Please select the type of equipment or Software package you are reporting about. For example: Printer/Copier/Scanner or Laptop, etc.
  • Description: Provide a full explanation of the issue, plus attach any screenshots of the error that may be helpful in the trouble shooting process.
